PATIENT ACCESS REPRESENTATIVE/RECEPTIONIST - DENTAL - $5,000 SIGN ON BONUS
JOB SUMMARY:
The Promise Patient Access Representative/Receptionist ensures that all persons contacting the clinic via phone or in person will be greeted and will receive prompt customer service. The Promise Dental Scheduler is responsible for a range of office duties that support the dental (and other) programs at Promise CHC.
DUTIES AND RESPONSIBILITIES:
- Greet and serve all patients as they arrive.
- Schedules, confirms, and registers patients.
- Answers patient questions regarding billing, insurance, Medicaid, health care concerns, as well as establishes financial eligibility and relays information to appropriate program staff.
- Refers patients to the outreach staff for assistance as needed with Medicaid, hawk-I, marketplace, and other community resources or patient assistance programs as needed.
- Provides accurate information to callers and the public about Promise CHC services & fees.
- Obtains and verifies demographic data from patients as they enter the facility. Assures the confidentiality and accuracy of patient’s financial and health record.
- Records receipts of payment from patient into the system. Handles petty cash for facility. Accounts for cash received prior to end of shift, reconciles cash drawer and prepares for deposit.
- Performs other job duties as assigned.

JOB SUMMARY:
The dental assistant provides dental care, dental office support, entry of data and clinical notes into the electronic dental record for the dental/oral health program. The dental assistant works cooperatively with health center dental director and other members of the dental staff to provide comprehensive oral health care to the patients of Promise Community Health Center. (Bilingual preferred, not mandatory)
DUTIES AND RESPONSIBILITES:
- Prepares operatory and dental clinic areas for treatment activities by placing necessary instruments, supplies and equipment for the procedure/dental care to be provided.
- Cleans, disinfects, sterilizes and maintains dental instruments and equipment. Performs minor equipment repair; replenishes clinic supplies. Maintains list of equipment for maintenance and instrument/supply inventory.
- Assists dental providers at chair side in providing treatment to patients with varying age-related, dental health, psychosocial and cultural needs.
- Prepares, maintains, and completes patient records consistent with clinic policies and procedure. dentist and hygienist schedules and registration rosters.
- Performs direct patient care as delegated by the dentist and in accordance with the state practice act, including tooth polishing, vital sign measurement, fluoride application, primary impressions, lab support, and x-rays.
- Provides patient support through scheduling of appointments, responding to phone calls, and overall education of treatment.
- Maintains the electronic dental health record system, ensuring that data is entered accurately and completely to meet current standards of care.
- Provides documentation for third party dental claims and assists in following up on outstanding claims as requested.
- Attends trainings and workshops as needed to remain current in the field of dental assisting. Shares and disseminates information from trainings and workshops with dental health team.
- Performs other necessary duties as assigned.

JOB SUMMARY:
Promise Dental Hygienist will provide preventive, screening, and diagnostic dental health services to the Promise target population with a goal of reducing barriers and improving the oral health of the patients/families served by Promise.
QUALIFICATIONS:
Current licensure as a dental hygienist in the State of Iowa. Professional dental hygiene experience in public health services preferred. Must be able to work well with a diverse population of persons, as well as individuals with low income.
DUTIES AND RESPONSIBILITIES:
- Provide patient screening procedures, conduct assessment of oral health conditions; review medical history to include oral cancer screening, head and neck inspection.
- Complete dental charting/documentation of all procedures and assessments.
- Complete and develop dental radiographs.
- Perform removal of hard and soft deposits on surfaces of teeth.
- Apply preventive materials to teeth, including sealants and fluoride varnishes.
- Provide age appropriate teaching of oral hygiene and other measures to maintain oral health.
- Counsel patients regarding good nutrition and oral health.
- Makes dental impressions of patient’s teeth if indicated/required.
- Ensure completion of a risk assessment on all newly eligible Medicaid-enrolled children under age 18; work with child health coordinators to develop protocols for informing newly-eligible Medicaid enrollees about dental services.
- Assist with office management activities as required for a dental/oral health service, including but not limited to the following:
- Assist families in setting up and keeping appointments.
- Refer patients who need assistance with transportation.
- Ensure that a referral/recall system is in place and utilized.
- Remind families when they are due for appointments.
- Provide oral health education.
- Refer or assist families in finding payment sources for dental care.
- Assure data entry/billing records of oral health services.
- Order and stock dental supplies and oral health educational materials.
- Attend and participate in state oral health meetings, staff meetings, and other conferences which enhance training and skills.
- Assist dental director in developing Promise oral health policies and providing reports to Promise Board.
- Provide training and oversight for other staff involved with oral health services.

DENTIST
JOB SUMMARY:
Responsible for providing dental services to the target population of Promise in accordance with the Iowa Dental Practice Act and Iowa Department of Public Health Dental Supervision requirements.
DUTIES AND RESPONSIBILITIES:
- Diagnoses and treats problems with teeth and tissues in the mouth, along with giving advice and administering care to help prevent future problems.
- Develops dental hygiene treatment plans and sequences treatment to enable patients with diverse back grounds to receive comprehensive dental hygiene services.
- Provides comprehensive dental services.
- Provides education to the patient and family regarding assessment data, treatment plan pre- and post-therapy instructions, and prevention of future disease and as appropriate for the patient’s age, educational level, physical and mental condition and cultural background.
- Develops and maintains patient education material for use at chair side and for office display.
- Provides instruction on diet, brushing, flossing, the use of fluorides, and other aspects of dental care.
- Maintains and completes records of dental patients, consistent with clinic policy.
- Assists the program director with implementation of the dental community outreach program.
- Assists with business office procedures, as needed and as patient schedule permits.
- Attends continuing education programs as required by license and to maintain skills.
- Attends and participates in staff meetings, in-service programs and Promise events regularly.

FAMILY MEDICINE PHYSICIAN
JOB SUMMARY:
Physician will provide primary health care services to patients and families in accordance with health care plans mutually established by provider and patient, and will follow policies, procedures and protocols established by Promise.
DUTIES AND RESPONSIBILITIES:
- Diagnose and treat health problems and injuries by conducting physical examinations with a focus on preventative care.
- Orders and interprets diagnostic studies to assist with diagnosing, treating, and monitoring chronic and acute diseases.
- Conducts developmental screening and immunizations for well child care, utilizing tools/screening instruments as per FQHC standards of care.
- Designs, implements, and evaluates plans of care resulting from assessment of physical, psychological, emotional, societal, and environmental needs of patients.
- Demonstrates and promotes the role of physician and Promise through teaching, modeling, educating, and counseling of preventative health services utilizing self-care skills.
- Exhibits knowledge of FQHC Health Care measures by meeting service productivity and performance standards aligned with FQHC guidelines while seeking opportunities to promote Promise’s mission through outreach.
- Completes timely and essential correspondence, phone calls, and dictated letters to patients and referring physicians in keeping with patient-centered medical home standards, using standard documentation of medical records as reference.
- Improves patient outcomes by utilizing evidence-based guidelines and participating in quality improvement activities to enhance quality care, patient satisfaction, and staff morale.
- Completes and submits charge tickets for daily services; complies with the billing process, policies and visit standards necessary to secure 3rd party and patient reimbursement for health care services rendered.
- Attends Promise meetings and in-service offerings, and participates in assigned Promise committees and events.
- Pursues educational opportunities to improve knowledge and skills resulting in superior patient care.
