Promise Community Health Center opened in Sioux Center in 2008 and operates as the only Federally Qualified Health Center serving the far northwest corner of Iowa.


We partner with our patients to provide comprehensive, team-based health care. We value your relationship with your personal care team, which includes your primary care provider, a nurse, and other clinical staff who serve you based on your needs. This team helps you strive for a better state of health through discussing available resources, encouraging you to set personal goals, and offering high-quality health care.
Promise Community Health Center Family Practice Medical Team includes five providers, Dr. Del Lassen, M.D., Medical Director, and advanced registered Nurse Practitioners, Tana Kass, Emily Leinen, Sarah Hietbrink, Stacy Schaap, and Roleen Walgenbach.
The team also includes registered nurses and medical assistants, also interpreters for spanish-speaking patients are integrated as part of each provider team.

ACUTE CARE
We offer care for acute illness and urgent problems. Our medical care staff will plan your treatment and complete any procedures or tests that are needed. Our providers work with area hospitals and follow patients through their entire treatment if they require services outside of our facility.

CHRONIC DISEASE MANAGEMENT
Whether you are newly diagnosed with a condition or have been diagnosed for many years, our staff provides a model of care unique to your needs. We offer medication management, goal setting and coaching for you and your family, health assessments, and personal education. Our medical care team coordinates patient care for conditions such as cardiovascular disease, chronic respiratory disease, and diabetes.
IN-HOUSE LABS
Our health center performs many patient laboratory tests on-site in a Clinical Laboratory Improvement Amendments (CLIA) waived lab. For any tests we are unable to complete on-site, staff will coordinate testing at another facility and will follow up with patient’s results, in most cases the next day.

IMMUNIZATIONS AND PREVENTIVE CARE
Our medical care team is committed to offering preventive care, immunizations, and educational tools to help you manage your health. We help patients understand risk factors and early disease warning signs to prevent illness and disease. We also provide vaccinations for adults and children, including free vaccinations for newborns to 18-year-olds who are eligible for the Vaccines for Children (VFC) program.
WELL-CHILD CARE
We care deeply about your child’s health and growth, and we are well-equipped to offer the care your family needs. Our staff is trained in the care of patients from newborns through adolescents and can provide all physicals, immunizations, acute and urgent care you need along the way. It is important for your child to be seen regularly by their care team — even if they are not sick — to ensure they are growing and developing as expected.

WELL-WOMAN EXAMS
Our providers offer care for every stage in a woman’s life. Preventive care is the focus of our well-woman exams through clinical breast exams, Pap smears, complete physicals, and laboratory work. Promise is committed to answering your questions and taking time to learn about your health.
FREE PREGNANCY TESTING
Think you might be pregnant? Promise offers free and confidential pregnancy testing. Results are available immediately and we can help you from there.
Call Promise to schedule an appointment for free pregnancy testing and meet with a Promise Nurse for next steps. Your plan may include scheduling an appointment with our Promise midwife providers for prenatal care or fertility counseling. If you don’t wish to become pregnant right now, your plan can include scheduling an appointment with our providers for birth control options.

IOWA CARE FOR YOURSELF PROGRAM
Our medical care staff can assist eligible women with enrolling in the Iowa Care for Yourself Program. This program provides funding to low-income women for breast and cervical cancer screenings, including an office visit, mammogram, and, if eligible, a Pap test.
FAMILY PLANNING SERVICES
Promise offers a variety of options for women seeking family planning methods and counseling. Our providers will discuss your current situation and what options are best for you. Call 712-722-1700 to schedule a counseling visit with one of our providers.
STD SCREENING
We are one of 65 designated sites for the Iowa Department of Public Health’s Sexually Transmitted Diseases Program. Through this program, we can offer affordable STD testing, treatment, and counseling to individuals.

DOT AND IMMIGRATION PHYSICALS
Promise Community Health Center provides DOT physicals for commercial motor vehicle (CMV) drivers. All CMV drivers must have a DOT physical examination completed by a licensed medical examiner to obtain a commercial driver’s license (CDL). The DOT medical examiner’s certificate is valid for up to 24 months. DOT physicals are provided by Roleen Walgenbach, ARNP who enjoys providing this preventive service for the community.
Promise Community Health Center provides immigration physicals for those that require this for their United States citizenship or United States green card. Dr. Delwyn Lassen is designated as a civil surgeon and is authorized to complete an immigration exam. The medical examination entails a review of your medical history, physical examination, and testing for communicable disease.
