Promise Community Health Center believes that health begins in your own environment – in your home, in your workplace or school. The Population Health nurse health coaches serve as a resource, care manager, advocate and teacher for individuals, providers and the broader community.
Our Promise Nurse health coaches work one-on-one with individuals and their families in the clinic setting to evaluate health needs, environment and barriers to better health. In doing so, they help their patients establish a plan and guide them and their families along the way to achieve the plan. They also work closely with local and state resources to ensure that each person receives the support that they need.
Promise Community Health Center Population Health Team consists of 3 team members. Amanda Rickard is the Population Health Team Manager, Lorilyn makes up the Health Coach Nurse on the team and Ruth is our Patient Navigator. Also Kailee Jenness is a social worker and Victor Garcia is our Transportation Specialist.
