DATA AND QUALITY SPECIALIST
JOB SUMMARY
The Data and Quality Specialist provides support to the health center’s quality improvement and risk management program. The Data and Quality Specialist supports coordination among all services and departments of the health center.
DUTIES AND RESPONSIBILITIES:
The Data and Quality Specialist will work closely with the Director of Quality & Program Development and members of the QI team to complete quality improvement/risk management activities. This position will serve as a resource to Promise staff with regard to Promise’s EHR and IT systems including workflows and troubleshooting. Duties will include assisting with running reports, data analysis, process improvement, updates to teams, survey administration, and training.
QUALIFICATIONS:
- Bachelor’s Degree or equivalent is preferred.
- High degree of computer literacy required, proficient in Microsoft and Outlook products.
- Capable of working with computer software and data analytics.

JOB SUMMARY
DUTIES AND RESPONSIBILITIES:
The Environmental Services Specialist is responsible for maintaining a safe and sanitary environment by following infection control, safety, and procedural guidelines in assigned areas which may include, but not be limited to, patient rooms, offices, nurse’s stations, hallways, bathrooms, break room, and lobby areas.
QUALIFICATIONS:
- Minimum of six months housekeeping or commercial cleaning experience preferred.
- Knowledge of materials, equipment, cleaning methods and cleaning chemicals is helpful.
- Strong customer service, communication and interpersonal skills are required.
- Previous custodial experience in a healthcare environment is preferred.

JOB SUMMARY
Job Summary: The Promise Clinic Nurse provides care to patients and families in accordance with the health care plan, policies, procedures and protocols established by Promise.
Duties and Responsibilities:
- Assesses plans, advocates for and provides health care for patients and community in accordance with Promise standards and policies, with an emphasis on prevention.
- Greets and escorts patients and families to exam rooms for provider visits. Performs data collection and documents subjective and objective data in patient records. Reviews health history with a focus on environment, self-care and home/community needs.
- Provides care management by serving in the role of patient advocate, serves as a resource by listening, attending to, and recording relevant patient information.
- Administers meds when needed and performs provider-ordered tests. Seeks opportunities to promote health and wellness with patient education and screening tools. (i.e. Ages and Stages, Depression screening)
- Remains calm and performs effectively during peak periods and during stressful times.
- Schedules and tracks referrals. Effectively utilizes community resources, informing and referring patients and families as needed.
- Anticipates and prepares exam room for provider(s) when needed, capturing teachable moments wherever and whenever possible.
- Utilizes universal precautionaries in obtaining specimens and in delivering care.
- Functions as a member of the health care team. Assists with orientation and education of new employees, ensuring their understanding of health center standards and performance measures, etc.
- Articulates the mission of Promise and seeks opportunities to promote the center through outreach. Strives to advance PCMH (Patient Centered Medical Home model with patients and families).
- Regularly attends Promise staff meetings, in-services and trainings. Understands FQHC UDS Performance measures and importance of Promise meeting these measures.
- Conscientiously documents all nursing care; communicates professionally, and honors patient confidentiality 100% of the time.
- Volunteers for service/participation in Promise events regularly.
- Meets FQHC productivity and performance standards.
- Demonstrates and supports service excellence standards.
- Completes required in-services and certifications 100% of the time.

Qualifications, Education and Experience:
- Current License to practice Nursing in the State of Iowa.
- Ability to communicate effectively orally and in writing; is computer literate.
- One to two years of nursing experience preferred. Experience in Community Health and/or Community Health Center preferred.
- Bilingual (Spanish speaking) preferred.
- Must have the ability to work as a team member, establishing effective relationships with patients and staff. Must be able to work well with low income populations in a respectful manner.

JOB SUMMARY
Promise Dental Hygienist will provide preventive, screening, and diagnostic dental health services to the Promise target population with a goal of reducing barriers and improving the oral health of the patients/families served by Promise.
QUALIFICATIONS
Current licensure as a dental hygienist in the State of Iowa. Professional dental hygiene experience in public health services preferred. Must be able to work well with a diverse population of persons, as well as individuals with low income.
DUTIES & RESPONSIBILITIES
- Provide patient screening procedures, conduct assessment of oral health conditions; review medical history to include oral cancer screening, head and neck inspection.
- Complete dental charting/documentation of all procedures and assessments.
- Complete and develop dental radiographs.
- Perform removal of hard and soft deposits on surfaces of teeth.
- Apply preventive materials to teeth, including sealants and fluoride varnishes.
- Provide age appropriate teaching of oral hygiene and other measures to maintain oral health.
- Counsel patients regarding good nutrition and oral health.
- Makes dental impressions of patient’s teeth if indicated/required.
- Ensure completion of a risk assessment on all newly eligible Medicaid-enrolled children under age 18; work with child health coordinators to develop protocols for informing newly-eligible Medicaid enrollees about dental services.
- Assist with office management activities as required for a dental/oral health service, including but not limited to the following:
- Assist families in setting up and keeping appointments.
- Refer patients who need assistance with transportation.
- Ensure that a referral/recall system is in place and utilized.
- Remind families when they are due for appointments.
- Provide oral health education.
- Refer or assist families in finding payment sources for dental care.
- Assure data entry/billing records of oral health services.
- Order and stock dental supplies and oral health educational materials.
- Attend and participate in state oral health meetings, staff meetings, and other conferences which enhance training and skills.
- Assist dental director in developing Promise oral health policies and providing reports to Promise Board.
- Provide training and oversight for other staff involved with oral health services.
- Participate in oral health quality improvement activities.
- Perform above duties as may be assigned.

Promise Community Health Center is an approved National Health Service Corps site. Primary care medical, dental and behavioral health clinicians can receive repayment of loans in exchange for a two-year commitment to work at an approved NHSC site, such as Promise, that reaches under-served populations in high-need areas. Promise has had several staff members participate in the NHSC program throughout the years.
