Promise Community Health Center is proud to announce it has been recognized as a 2021 Gold Awardee for Quality Performance in Top 10% among Health Centers!
The Community Health Quality Recognition (CHQR) badges recognize Health Center Program awardees that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology for the most recent UDS reporting period.
A large part of the quality performance measurement is also focused on preventive care and screenings, along with health outcomes for patients with chronic conditions such as diabetes and hypertension.

Promise Community Health Center also received the Advancing Health Information Technology (HIT) for Quality Award: Recognizing health centers that meet all criteria to optimize HIT services that advance telehealth, patient engagement, interoperability, and collection of social determinants of health to increase access to care and advance quality of care.
