
Promise Community Health Center is proud to share that our Hypertension Control Program has been featured in Rural Health Models & Innovations, a national collection of successful rural health programs maintained by the Rural Health Information Hub (RHIhub).
This recognition highlights the work our team is doing right here in Sioux Center to improve heart health outcomes across rural northwest Iowa.

Improving Blood Pressure Control in Rural Iowa
Promise Community Health Center launched a focused quality improvement initiative to help patients better manage hypertension. Through a team-based approach, our providers, nurse health coaches, medical assistants, pharmacists, and support staff work collaboratively to ensure patients receive consistent, accessible care.
Our program includes:
- Monthly follow-ups for patients with uncontrolled hypertension
- Ongoing support for patients with controlled blood pressure
- Self-measured and remote blood pressure monitoring
- Nurse health coaching focused on lifestyle and heart-healthy habits
- CDC educational resources on accurate blood pressure readings and low-sodium diets
- Bilingual scheduling and medical services in English and Spanish
Since launching these improvements, Promise has increased its hypertension control rate from 73% in 2022 to 84% in 2024, with 238 of 284 patients achieving controlled blood pressure. Over half of our patients with hypertension have received personalized support from a health coach.
Addressing Rural Barriers to Care
As a Federally Qualified Health Center serving a rural community, we understand the challenges our patients face — including transportation limitations, long travel distances, and financial concerns.
To make consistent care possible, Promise:
- Utilizes a transportation program to assist patients
- Offers free health coach or lab-only visits as one-month follow-ups
- Loans calibrated blood pressure cuffs for home monitoring
- Uses secure texting tools to stay connected between visits
- These patient-centered solutions have made a measurable difference in outcomes.
National Recognition for Local Impact
RHIhub selected Promise’s Hypertension Control Program for inclusion in its Rural Health Models & Innovations collection because of its innovative approach and demonstrated impact in a rural setting.
RHIhub is a national online information center focused on rural health issues and is funded by the Federal Office of Rural Health Policy. Its website includes more than 18,000 resources specific to rural healthcare. The Rural Health Models & Innovations section features successful programs from across the country that are improving health outcomes for rural residents.
