Population Health at Promise Community Health Center
The Population Health team at Promise Community Health Center shapes its program around the shifting health needs of the community, tailoring its approach to people rather than merely following guidelines.
“I will have the opportunity to reach out more to the communities we serve, finding new and creative ways to educate and promote healthy lifestyles throughout Northwest lowa,” Amanda explains.
This is achieved by reducing barriers to health access, providing education, and creating opportunities for making healthy lifestyle changes.
