The Addink Community Service Award is presented annually to an individual, team, business, ministry or family, who has made a positive difference in communities through volunteering, leading, and impacting lives.
The deadline to submit a nomination is October 1, 2023
This year’s honoree will be presented the award during Promise Community Health Center’s 14th annual “An Evening of Promise” celebration on Thursday, Oct. 19, at Terrace View Event Center in Sioux Center. The honoree will be notified prior to the event and recognized publicly.

The doors open at 5:30 p.m. and the event will begin at 6 p.m. The event will feature a catered dinner, entertainment by The Dueling Duo, dueling pianos, and the presentation of the Addink Community Service Award.
