The Family Practice Medical Team at Promise Community Health Center is led by Dr. Daniel Vanden Bosch, M.D., our Medical Director, and features a skilled group of advanced registered nurse practitioners: Sarah Hietbrink, Stacy Schaap, Mindy De Jager, and Wyatt Cook. Emily Leinen, who specializes as both a Psychiatric Mental Health and Family Nurse Practitioner, also plays a vital role in our team.
Supporting our providers, each medical team is composed of registered nurses and medical assistants to ensure comprehensive care. For our Spanish-speaking patients, interpreters are seamlessly integrated into each team to provide clear and effective communication.

Promise Community Health Center’s Prenatal/OB team includes two certified nurse midwives, Audra DeGroot, and Kari Ney. The team also includes registered nurses, medical assistants, and interpreters.
The Midwifery Care Team offers personal care tailored to you and your family throughout your pregnancy and after the birth of your child. Our midwives are experienced providers in pregnancy and childbirth.
