Job Summary: The transportation coordinator/float interpreter is responsible to provide safe, timely transportation to patients of all ages, gender, cultures, and backgrounds to and from Promise.  In addition, interpreting duties across departments are performed as needed.

FULL TIME – 30-36 HOURS

SALARY RANGE: Hourly Position

REPORTS TO: Population Health Manager

DUTIES AND RESPONSIBILITIES:

1. Complies with all parts of the Promise CHC passenger transportation safety policy.
2. Reviews and manages the patient transportation schedule.  Provides transportation and ensures timely arrival to and from appointments at Promise CHC.
3. Communicates with patients regarding schedule, confirms appointment times, and ensures safety at all times.
4. Performs vehicle inspection with accurate documentation, as per the transportation policy, daily when providing services.
5. Obtains consent for transport prior to providing services.  Must be done for each date that transportation services are provided.
6. Greets and escorts patients within health center to departments as appropriate for visit.  Interprets across departments as needed; including but not limited to dental, medical, behavioral health, vision, and population health.
7. Performs other job duties as assigned.

EDUCATION & EXPERIENCE:

• Bilingual in English and Spanish.
• Consistent demonstration of a team approach to care.
• Excellent Communicator.
• Valid drivers license with clean driving record.

PHYSICAL REQUIREMENTS:

• While performing the duties of this job the employee may be required to stand for long periods of time.
