Whether you are newly diagnosed with a condition or have been diagnosed for many years, our staff provides a model of care unique to your needs. We offer medication management, goal setting, and coaching for you and your family, health assessments, and personal education. Our medical care team coordinates patient care for conditions such as cardiovascular disease, chronic respiratory disease, and diabetes.
Our health center performs many patient laboratory tests on-site in a Clinical Laboratory Improvement Amendments (CLIA) waived lab. For any tests we are unable to complete on-site, staff will coordinate testing at another facility and will follow up with the patient’s results, in most cases the next day.

Our medical care team is committed to offering preventive care, immunizations, educational tools, and more to help you manage your health. We help patients understand risk factors and early disease warning signs to prevent illness and disease. We also provide vaccinations for adults and children, including free vaccinations for newborns to 18-year-olds who are eligible for the Vaccines for Children (VFC) program.
We care deeply about your child’s health and growth, and we are well-equipped to offer the care your family needs. Promise Community Health Center provides DOT physicals for commercial motor vehicle (CMV) drivers. All CMV drivers must have a DOT physical examination completed by a licensed medical examiner to obtain a commercial driver’s license (CDL). The DOT medical examiner’s certificate is valid for up to 24 months.
Promise Community Health Center provides immigration physicals for those who require this for their United States citizenship or United States green card. Dr. Delwyn Lassen is designated as a civil surgeon and is authorized to complete an immigration exam. The medical examination entails a review of your medical history, physical examination, and testing for communicable diseases.
