Promise Community Health Center Social Work Team meets one on one with individuals to aid them in meeting a variety of needs. These individuals are given support, information, and assistance while navigating resources, filling out paperwork, managing stress, and much more.


Kailee Jenness, the Promise Community Health Center Bilingual Social Worker works closely with different department teams at Promise. Kailee works with patients on a variety of stressors including crisis intervention, filling out paper work, safety planning, stress management and so much more.

CRISIS INTERVENTION
A crisis can be described as a disruption that cannot be solved by one’s own resources or skills, this crisis tends to disrupt a person’s or family’s normal or usual pattern of everyday life. It can include a situation that impacts your finances, family, housing, etc. Part of our social worker’s job includes crisis intervention. Crisis intervention includes but is not limited to, planning, implementation and finding available resources that may be able to assist a patient in a crisis situation.

SAFETY PLANNING
A safety plan is a plan developed collaboratively with a patient to identify triggers, symptoms, support, safe places, and goals. This safety plan is given to the patient to use as a resource for when they enter into a crisis state. As a social worker, Kailee has developed skills to work with individuals to develop their own safety plan.

REVIEW SOCIAL DETERMINANTS OF HEALTH
According to the U.S Department of Human Services, social determinants of health (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.
Examples of SDOH include:
- Not having safe housing and/or transportation
- Dealing with racism, discrimination, and/or violence
- Low access to education and/or job opportunities
- Low access to nutritious foods and/or physical activity opportunities
- Low language and/or literacy skills

Our social worker helps people cope with challenges in every stage of their lives. She analyzes clients’ backgrounds and situations to understand their needs, as well as their strengths and weaknesses to find the best ways to cope with stressful circumstances.
