Promise Community Health Center has achieved a significant milestone in its commitment to patient health by being recognized as a 2024 Million Hearts® Hypertension Control Champion!
This award, presented by the Centers for Disease Control and Prevention (CDC) and the Million Hearts® initiative, celebrates healthcare organizations that have achieved exceptional rates of hypertension control, helping patients maintain healthy blood pressure levels and prevent heart attacks and strokes.
Being recognized as a Million Hearts Hypertension Control Champion is an incredible honor and a true reflection of the dedication and hard work of our team. The Million Hearts® initiative recognizes clinicians, practices, and health systems that demonstrate excellence in managing hypertension. To be recognized as a Champion, organizations must achieve a hypertension control rate of at least 80% among their patients 18-85 years old with hypertension. Promise Community Health Center is one of only four Iowa organizations who has been given this recognition.
This award is part of Million Hearts’ goal to prevent 1 million heart attacks and strokes within five years by promoting effective blood pressure management across the nation. The initiative encourages healthcare providers to implement strategies that improve patient outcomes, reduce health disparities, and promote heart health.
