At Promise Community Health Center, we believe one of our greatest strengths is our people. We are proud to recognize and celebrate several team members who have recently stepped into new roles and leadership positions. Their dedication, passion, and commitment to serving our patients have made a lasting impact, and we are excited to see them continue to grow within the Promise family.
Please join us in congratulating the following team members on their well-deserved role changes:

Kim Davelaar, Quality and Population Health Coordinator
Over the past 12 years, Kim has served in several roles throughout the organization, including Clinic Nurse, Population Health RN and Diabetes Educator, Quality Coordinator, and now Quality & Population Health Coordinator. Through these roles, she has helped improve clinical quality measures, strengthen population health programs, support care teams, and enhance patient outcomes. Her experience across multiple areas of the organization has provided valuable insight into both clinical care and quality improvement.
In this role, Kim will continue leading quality initiatives, supporting clinical teams in achieving quality goals, monitoring performance measures, and identifying opportunities to improve patient outcomes through a population health lens. She will also continue collaborating across departments to further integrate population health strategies into patient care while advancing Promise’s commitment to delivering high-quality, patient-centered care.
After serving as Women’s Health Coordinator, Jenna has transitioned into the role of Clinical Administrative Coordinator. During her time in Women’s Health, she became deeply invested in supporting the OB team, patients, and clinical staff while gaining valuable experience in clinic operations. This transition allows her to build on that experience by supporting not only the OB department but the medical clinic as a whole.
In her new role, Jenna will support the medical department through workflow coordination, staff support, scheduling, project management, and operational initiatives that help ensure the clinic runs efficiently. She will also collaborate with clinical teams to improve processes and support high-quality patient care. Jenna looks forward to continuing to grow professionally, taking on new challenges, and contributing to Promise’s mission by supporting both staff and patients through effective clinical operations.

Jennifer Plathe, Mental Health Department Supervisor
Jennifer’s transition from Mental Health Therapist to Mental Health Supervisor reflects a natural progression into a formal leadership role within the Mental Health Department. While continuing to provide direct therapy services, her expanded role allows her to focus on supporting the department through day-to-day leadership, staff support, policy and workflow development, onboarding and training, and ensuring the continued delivery of high-quality, patient-centered care.
Jennifer also serves as a bridge between the Mental Health team and organizational leadership, supporting communication and collaboration as the department continues to grow. Remaining involved in direct patient care allows her to stay connected to the experiences and needs of both patients and staff. She looks forward to continuing to support the team and identify opportunities to strengthen and expand mental health services within the organization.
Over the past two years, Dr. Gigi has provided leadership and expertise as a consultant focused on the dental department. Through her partnership, she has helped strengthen department operations by evaluating and redesigning workflows, improving scheduling processes, and supporting staffing structures. We are incredibly grateful for the advancements she has helped achieve.
In this expanded role, Dr. Gigi will continue supporting the dental department as a priority area over the next six months as we continue onboarding new team members, refining workflows, and strengthening department systems. At the same time, she will begin extending her expertise across the health center to support broader organizational goals as our Leadership Development and Clinical Advisor.
