Are you ready to quit smoking?
Did you know that Promise Community Health Center has a program to help you quit smoking. Nurse health coaches can meet you in person, over the phone or through telehealth (video call). Schedule an appointment with a provider at PCHC for an initial assessment and then pick the option that fits best for you:
- Talk to a nurse health coach by phone or in person to learn ways to improve your success. This would include the 1st meeting and then 1-3 meetings between medical visits to help you reach your goals.
- Talk to a nurse health coach in person to learn tips to quit smoking and quit smoking products such as nicotine patch, lozenge and gum to improve your success. PCHC provides quit smoking products at a reduced rate for those who qualify for the program. Meet with a nurse health coach monthly to discuss how quitting smoking and the products are working.
