Promise received the GOLD award for being a Health Center Quality Leader! We also received four additional badges, Health Disparities Reducer, Advancing HIT, Addressing Social Risk Factors to Health, and Access Enhancer! This means we are in the top 10% for all health centers NATIONWIDE for our quality measures!
Iowa’s community health centers are at the forefront of promoting the well-being of patients across the nation. Annually, the Health Resources and Services Administration (HRSA) acknowledges the remarkable accomplishments of Health Center Program recipients. These achievements span diverse areas such as quality, access, health equity, health information technology, and COVID-19 public health emergency response for the preceding calendar year. Promise Community Health Center has received multiple Community Health Quality Recognition (CHQR) badges in 2023. Promise was also the #1 health center in Iowa for 2 measures: dental sealants & statin therapy and we were above the Iowa average in almost all quality measures!
The CHQR badges recognize Health Center Programs who have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology. A substantial part of the quality performance measurement is also focused on preventive care and screenings, along with health outcomes for patients with chronic conditions such as diabetes and hypertension.

Promise Community Health Center also received the Advancing Health Information Technology (HIT) for Quality Award: Recognizing health centers that meet all criteria to optimize HIT services using tools such as telehealth, South Dakota Health Link, automated messaging, and more to increase access to care and advance quality of care.
Promise Community Health Center received three additional quality recognition badges. We received the Access Enhancer badge in recognition of our increased total number of patients and the number of patients who receive at least one comprehensive service (mental health, substance abuse, vision, dental, and/or enabling) by at least 5%. Promise has increased in this area by 27% since 2020.
The Health Disparities Reducer badge demonstrates at least a 10% improvement in low birth weight, hypertension control, and/or uncontrolled diabetes clinical quality measures for certain racial or ethnic groups.
The Addressing Social Risk Factors badge recognizes health centers that are screening and responding to PRAPARE patients’ assessments for social risk factors impacting patient health and are increasing access to enabling services.
