The Promise Community Health Center Transportation Program has officially kicked off in the new year!
Promise Community Health Center is constantly reviewing and assessing social determinants of health (SDOH), which are the conditions in the environments where people are born, live, learn, work, play and worship that affect a wide range of health, functioning, and quality-of-life outcomes and risks. Approximately 40% of a person’s health outcomes are determined by their SDOH’s.
Here at Promise through active SDOH screenings, patients are asked about their access to food, clothing, transportation, housing, daycare, close relationships, stressors and more. These screenings allow the teams at Promise to not only provide the highest quality of care for your medical needs, but also to identify the inequities present and gives us the opportunity to provide resources, connections and support for our patient’s emotional, mental and social health.
Here at the health center we work with families and individuals daily who know first-hand the realities of these inequities. Transportation is something many of us don’t actively think about until the low fuel or check engine lights flash; however, lack of public transportation, rural living and bitter winter weather make accessible transportation a first-hand challenge for many in our community.
The Promise Community Health Center leadership team, staff, and board of directors, identified transportation as a significant barrier in our community, prompting the establishment of a transportation program within the heath center.
Promise is beyond excited to start this program and offer transportation services to our patients. We are hopeful that this will be a blessing for many and will begin to bridge the gap of accessible health care for all. Victor Garcia, joined the population health team in October 2021 in the role of transportation coordinator and float interpreter. Victor, a Northwestern college graduate, is an undeniable fit at Promise. His warm, welcoming, and helping demeanor makes him the perfect person to welcome patients into their experience at the health center.
