This award recognizes the health center’s unwavering commitment to prioritizing blood pressure control and improving patient health outcomes. The Gold+ level recognition is given to practices that exhibit an exceptional dedication to measurement accuracy and demonstrate that 70% or more of their adult patients with high blood pressure have their condition under control. High blood pressure, or hypertension, is a prevalent and significant health concern in the United States. With 121.5 million adults living with hypertension—nearly half of the country’s adult population—effective diagnosis and management are crucial. Hypertension is a leading risk factor for heart attacks, strokes, and preventable deaths. Unfortunately, less than half of those with hypertension have their blood pressure under control, emphasizing the importance of initiatives like Target: BP.
