The Addink Community Service Award nominations are now open!
This is an annual award presented to an individual, team, business, ministry, or family who has made a meaningful and positive impact in their community. This award celebrates those who have gone above and beyond in volunteering, leading, and enriching the lives of others.
If you know someone or a group who has made a significant contribution to their community, now is the time to honor their efforts. The deadline for submitting nominations is September 20th, 2024.
To nominate a deserving candidate, please fill out the nomination form provided below. The honoree will be notified in advance of the event.
This year’s honoree will be celebrated during Promise Community Health Center’s 15th annual “An Evening of Promise” on Thursday, October 10th, 2024, at the Terrace View Event Center in Sioux Center. The evening will feature a catered dinner and entertainment by The Dueling Duo, a dynamic dueling pianos act that promises to provide an unforgettable experience. The event brings together community members for a night of celebration and entertainment.
