The Addink Community Service Award is presented annually to an individual, team, business, ministry or family, who has made a positive difference in communities through volunteering, leading, and impacting lives.
The deadline to submit a nomination is September 14, 2022.
This year’s honoree will be presented the award during Promise Community Health Center’s 13th annual “An Evening of Promise” celebration on Thursday, Oct. 27, at Terrace View Event Center in Sioux Center. The honoree will be notified prior to the event and recognized publicly.

The event will begin at 6 p.m. with doors opening at 5:30 p.m. The event will feature a catered dinner, entertainment by The Dueling Duo, dueling pianos, and presentation of the Addink Community Service Award.
Tickets are $50 for adults or $400 for a full table which seats 8. Click the button to purchase tickets for the event!
