Promise Community Health Center is proud to announce it has been recognized as a 2022 Gold Awardee for Quality Performance in Top 10% among Health Centers!
The Community Health Quality Recognition (CHQR) badges recognize Health Center Program awardees that have made notable quality improvement achievements in the areas of access, quality, health equity, and health information technology for the most recent UDS reporting period.
A large part of the quality performance measurement is also focused on preventive care and screenings, along with health outcomes for patients with chronic conditions such as diabetes and hypertension.

Promise Community Health Center also received the Advancing Health Information Technology (HIT) for Quality Award: Recognizing health centers that meet all criteria to optimize HIT services using tools such as telehealth, South Dakota Health Link, automated messaging, and more to increase access to care and advance quality of care.
Promise Community Health Center received three additional quality recognition badges. We received the Access Enhancer badge in recognition of our increased total number of patients and the number of patients who receive at least one comprehensive service (mental health, substance abuse, vision, dental, and/or enabling) by at least 5% during consecutive UDS reporting periods. Promise has increased in this area by 27% from 2020. The Health Disparities Reducer badge which demonstrates at least 10% improvement in low birth weight, hypertension control, and/or uncontrolled diabetes clinical quality measure for certain racial or ethnic groups. The Addressing Social Risk Factors badge recognizes health centers that are screening and responding to PRAPARE patients assessments for social risk factors impacting patient health and are increasing access to enabling services.
