At Promise Community Health Center we offer a discount program that applies to health center services. Eligibility for the sliding fee scale and discount amounts are determined by family size and income. Come in and talk with our Promise outreach team to see if you are eligible and answer any questions you might have.
Promise Community Health Center accepts both Medicaid and Hawk-I programs and we have a trained outreach team to assist with questions and applying for both programs.
Medicaid is a health insurance plan provided by the government that covers things like doctor’s visits, medicines, and hospital stays. All states have their own versions of Medicaid plans. These plans are for people with low income or who have disabilities. You have to qualify based on your income or sometimes based on your disability.
The Healthy and Well Kids in Iowa (hawk-i), federally known as Children’s Health Insurance Program (CHIP), provides health coverage to nearly eight million children and families with incomes too high to qualify for Medicaid, but can’t afford private coverage. In some states, CHIP covers parents and pregnant women. Each state offers CHIP coverage, and works closely with its state Medicaid program. Some benefits covered through CHIP, include:
- Routine check-ups
- Doctor visits
- Dental and vision care
- Laboratory Services
Our Promise Community Health Center Outreach Team can help assist with the federal government program food stamps or now known as SNAP. SNAP stands for the Supplemental Nutrition Assistance Program. SNAP is a federal program that helps millions of low-income Americans put food on the table. Across the United States there are 9.5 million families with children on SNAP. It is the largest program working to fight hunger in America.
Our Outreach Team at Promise Community Health Center is very knowledgeable in the federal health insurance program called Medicare. Medicare is the federal health insurance program for:
- People who are 65 or older
- Certain younger people with disabilities
- People with End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant, sometimes called ESRD)
Over the years, Medicare has become the leading health care insurance program in the United States for seniors.
The Promise Community Health Center Outreach Team is very knowledgeable in many of the federal and state programs such as WIC and HUD. Promise Community Health Center is a Certified Application Counselor organization with trained staff available to assist you in enrolling in the Health Insurance Marketplace. Health Insurance Marketplace is a service that helps people shop for and enroll in affordable health insurance. Our Certified Application Counselors (CACs) will meet with you to discuss enrollment eligibility and find the best healthcare coverage for you and your family.
At Promise Community Health Center we do not want transportation to be a barrier to patients and their families getting medical care, vision care, dental care, behavioral healthcare or any other services needed. Our Outreach Team works closely with local programs and volunteers who can help with transportation needs.
