You will need to bring the following items for the vaccine:
- Insurance card
Once you have been vaccinated you will be required to remain for observation for 15 minutes with up to 30 minutes of observation being recommended for patients who have a history of severe allergic reactions from other vaccines, medications, foods, etc.
Promise Community Health Center understands that many want to receive the vaccine. Please be patient as we work through distribution. We will continue to update our communities through social media and our website as new information is available.
COVID-19 VACCINE BOOSTER DOSE INFORMATION
CDC now recommends that certain people are now eligible to receive a COVID-19 booster shot, including those who received Moderna and Johnson & Johnson/Janssen COVID-19 vaccines. A booster shot is now available for all three COVID-19 vaccines (Moderna, J&J and Pfizer) with the following criteria:
- The use of a single booster dose of the Moderna COVID-19 Vaccine that may be administered at least 6 months after completion of the primary series of Moderna or Pfizer COVID-19 vaccine or 2 months after a single dose of the Janssen (Johnson and Johnson) to individuals 18 years of age and older.
- The use of each of the available COVID-19 vaccines as a heterologous (or “mix and match”) booster dose in eligible individuals following completion of primary vaccination with a different available COVID-19 vaccine.
Promise Community Health Center is now administering doses of Moderna COVID-19 boosters on Mondays.

Vaccination plans in Sioux County are dependent on vaccination supply and may be subject to change. While vaccine supply is limited, priority for vaccination will be given to individuals who are the highest risk for negative outcomes of COVID-19. More information about vaccination plans will be available after the Iowa Department of Public Health (IDPH) releases more information about the number of doses that will be available in Sioux County.
We will continue to work with IDPH and other healthcare centers in Sioux County to determine the next steps in COVID-19 vaccine administration.
